Requests received by 3:00 PM will be replied to on the same day. All requests made after 3:00 PM will receive a reply by the end of the next business day. All replies will be by telephone, so please be sure to leave your telephone numberl.
Please note that we will need you to bring your new insurance card and a picture ID to your next appointment in order to complete a change of insurance request.
